Directions: Applications must be completed in full to be reviewed and approved. Upon submission, additional information may be required to complete processing of your application, such as proof of income, proof of address, and proof of identification. Applicants home address must be in the following zip codes to qualify for income-based scholarships: 46302, 46341, 46347, 46383, 46385. Work addresses will not be accepted. YOUR FORM IS NOT SUBMITTED UNTIL YOU SEE THE CONFIRMATION PAGE AFTER CLICKING "SUBMIT"!

Income verification can be submitted in the form of:

  • A W2/1040/1099 or similar most recent year-end tax document (most preferred)
  • At least 2 months of paystubs
  • Be sure to include any benefit letters for additional federal or state disbursements (TANF/WIC/SNAP/Disability/Social Security/Pension/Child Support/Alimony) 

 

Scholarship Usage and Account Policies 

To ensure the sustainability of our Income-Based Scholarship Program and support as many community members as possible, we have established the following policies:

Policy on Usage: Membership scholarships require a minimum of 5 visits per month to remain active. This ensures that recipients are utilizing their membership and benefiting from YMCA resources.

           If the visit minimum is not met: 

  • First Month: A reminder email will be sent as a courtesy.
  • Second Month: A follow-up phone call will be made to discuss any challenges or reasons for low usage.
  • Third Month: If the minimum is not met for three consecutive months, the scholarship will be suspended for 2 months. After this period, the applicant will need to reapply.

 

Policy on Recurring Payment Issues: For memberships with recurring payments, consistent failed payments (bounce backs) can impact account status.

  • After 3 consecutive failed payments: Membership will be terminated.

 

Policy on Outstanding Balances: All outstanding balances must be paid in full before activating a Financial Assistance (FA) approval. This ensures that all accounts are current and ready for membership activation.
